Support Planning

If you are thinking about the kind of support you might need to stay independent and well, you and your family might benefit from having a Support Plan.


You can draw up your own Support Plan using the guide below, or we can help you to develop a Support Plan that sets out what support you need, when you need it.


There are several key areas a good Support Plan should address:


  • What is important to you?

  • What do you want to change?

  • What do you want to stay the same?

  • What kind of support you need?

  • How will you get the support you need?

  • How will you pay for your support e.g. a personal budget from the local authority or NHS or self-funded?

  • How will you manage your support?

  • How will you stay in control of your life?

  • What are you going to do to make your plan happen?


Making Your Plan Happen


Once you have your Support Plan, you will need to make it happen. Putting support and services in place is sometimes called Brokerage. You can do this yourself by shopping around and speaking to your family and friends or you may want to use an organisation such as disAbility Cornwall & Isles of Scilly to help you. We can help cost the services you need and even put them in place for you.
