Helping people live well and safely with dementia.

Dementia Together Cornwall is part of the Cornwall Memory Support Pathway, providing personalised advice, connections, and practical support for people living with dementia and for their carers.

We’re here to help you stay independent, safe, and supported, at home and in your community. Whether you need trusted information, friendly guidance, or help finding the right local services, we’ll work alongside you to make living well with dementia a reality.

Together, we can build a community where everyone affected by dementia feels understood, included, and empowered.

We work closely with our partners across Cornwall to ensure that everyone can access support in their own community. Together, we make sure people know their local services and professionals who are there to help them.

Whether over the phone or face to face, we offer practical advice, guidance, and emotional support, as well as opportunities to connect with others living with dementia and caring.

The Pathway

Step 1

Visit your GP, they will do a full review and screening

Step 2

Appointment with the Memory Assessment Service for dementia diagnosis.

Step 3

Meet your local Primary Care Dementia Practitioner for post diagnostic support and a personalised health check.

Step 4

Contact your local community Dementia Together Team whenever you need. We are here for you.

This pathway shows healthcare professionals how people concerned about memory can move between high, medium, and low levels of clinical and community support, from GP assessment and Memory Assessment Services through to dementia advisors and family support, ensuring the right care is provided at each stage.
